Why does the add-in fail to load?

Solution:
 In Windows 2003 Server x64 bit environment, there is a missing Primary Interoperability Assemblies (PIA) package. This is a Microsoft Office 2007 component and is downloadable through the following link:

It is a Microsoft redistributable package. After installing the package, you will then need to manually enable the add-in from the Excel option.
